Take part in a free preventive care programme for survivors of acute coronary syndrome and learn how to take care of your heart.

- people aged between 15 and 65
- residents of the Świętokrzyskie Voivodeship
Acute coronary syndrome (ACS) is associated with 3 acute manifestations of coronary disease: heart attack (STEMI and NSTEMI) and unstable angina.
Participants are recruited by nurses/health educators at selected medical clinics in the Świętokrzyskie Voivodeship.

You are assigned a nurse/health educator.
For 12 months, you maintain contact with the nurse/educator (through phone calls and meetings where you will learn how to take care of your heart. Additionally, your basic details, i.e. your BMI and blood pressure, will be monitored during the meetings).
Over the course of the programme, your health will also be monitored during appointments with your GP.
After 12 months, your nurse/health educator will provide you with further, long-term recommendations, which will help you to enjoy a long life.
